What to Bring About Workers Compensation

A written summary of the accident or injury occurrence along with names of witnesses and a description of surrounding factors or company history that may have played a part in the accident or injury

Copies of any worker’s compensation claim forms

Copies of current medical records, detailing treatment for injury or disability.

Summary of medical expenses (including any out of pocket prescription expenses with receipts) and doctor visits as well as a copy of any medical reports, prescriptions and/or recommendations from your medical physician.

Copies of any workplace injury or accident reports

Copies of paystubs from all employer’s for the 13 weeks prior to your date of accident as well as any paystubs received from your employer and/or the insurance carrier received after your injury.
